Friends of the Michigan Libraries Survey

The Friends of Michigan Libraries (FOML) is undertaking a survey project to ascertain whether each Michigan library has its own Friends of the Library group. FOML would like this project to be a comprehensive survey including libraries of all types -- public (to include public library branches), academic, special, school, etc. Therefore FOML is requesting that ALL Michigan libraries respond to the following survey. FOML expects to assemble a directory of Michigan Friends from the data provided by each library's survey response.

The library director may wish to complete the survey or a Friends board member; most important, FOML wants to hear back from ALL Michigan libraries. Thank you in advance for your cooperation in this survey project.

This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven't) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
